We are Beechwood Dental (BWD) of 9 Dunville Avenue, Ranelagh, Dublin 6.
We are committed to respecting your privacy and to comply with the Data Protection and Privacy laws. The objective of this is to inform you how we manage your personal data. This privacy statement applies to all your personal data and sensitive medical information in our possession. Personal data is any information concerning a living individual who is or is likely to come into the possession of a data controller. Sensitive personal data for the purposes of this statement will include data relating to your physical condition and any connected medical information that you provide to us or that comes to our attention through the provision of health care. Data covered under this policy includes the following: name, age, address, occupation, contact details, medical history and physical health.
This privacy statement emphasizes that Data will be collected, processed and used by BWD on your behalf for the purposes of providing physiotherapy, medical care, health care, advice and treatment to you necessary for the performance of our contract with you. Your data will be used by us only in a manner consistent with the purposes for which we obtained it which is to provide you with the relevant services that you have sought our assistance with. Data may also be processed and used by other organisations on our behalf for the purpose of providing our services to you.
Any data received by us will be treated in the strictest confidence and all information will be collected and stored in a manner which does not compromise your identity. Any medical data retained by us will be subject to the highest standard of security and will not be disclosed to any third party without your knowledge and / or consent.
Information sharing
As outlined in this privacy statement, we only share your data with other medical personnel or individuals in the following circumstances: we have your consent, where any information provided is deemed to be sensitive personal data and we will require your explicit consent prior to sharing this information. We share your Data with our Practice Management System provider. We may also send you emails through our email service provider. They may only process this data for the purpose of providing us with their services and no other purpose.
Retention of Data
Data will be retained by us for any period required by Law and in order to market the services of our Practice.
Accuracy of Data
Any information you volunteer to us must be complete, accurate and up to date. You must immediately notify us if this information becomes out of date or inaccurate.
Access request and Rectification
Our privacy statement outlines the following requests and rectifications that can be made:
You have the right to request a copy of the Personal Data that we hold about you together with other information about our processing of that personal data.
You have the right to request that any inaccurate data that is held about you is corrected or if we have incomplete information you may request that we update the information such that it is complete.
You have the right to request us to delete Personal Data that we hold about you.
You have the right to request that we no longer process your Personal Data for particular purposes or to object to our processing of your Personal Data for a particular purpose.
You have the right to request us to provide you or a Third Party with a copy of your Personal Data in a structured commonly used machine readable format.
If we are processing Personal Data based on your consent you may withdraw that consent at any time. This does not affect the lawfulness of processing what took place prior to its withdrawal. If you are unhappy with how we process your Personal Data we ask you to contact us by way of our Data Controller so that we can rectify the situation.
You may lodge a complaint with a Supervisory Authority and the Irish Supervisory Authority is the Data Protection Commission.
We do not use any Personal Data for the purposes of automated decision making or profiling.
All access to information will be subject to the proper verification by you of your identity. BWD may require personal information from you as it deems reasonably necessary, in its sole discretion, to verify your identity. If you wish to obtain a copy of the data retained by us, please send a written request to BWD. We will respond to your request as soon as possible and we will take no longer than 1 month to process your request.
